Primary Job Responsibilities:

  • Conduct comprehensive patient assessments to evaluate their healthcare needs.
  • Develop, implement, and update individualized care plans based on patient assessments and preferences.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor and document patient progress and responses to treatment.
  • Educate patients and their families on health management, medication administration, and care plans.
  • Coordinate care with interdisciplinary team members to promote optimal health outcomes.
  • Ensure compliance with agency policies, state, and federal regulations.
  • Act as a patient advocate, promoting their rights and preferences in care.
