Responsibilities:
- Conduct initial client assessments and continuously refine care plans.
- Provide hands-on nursing care and patient education.
- Maintain open lines of communication with patients, families, and the healthcare team.
- Manage patient care coordination for your caseload.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
Qualifications:
- Licensed as a Registered Nurse / RN in the state you work in.
