Overview
Moments Hospice recognizes the important role of the Hospice Aide on the hospice team. The CNA Coordinator is a member of the office administrative staff and assists in other tasks as needed. The CNA coordinator will be responsible for the schedule and mapping of all CNA schedules.
Qualifications:
- HCHB EMR experience preferred
- Outpatient Scheduling Experience required
- Certified in state(s) practicing as a Nurse’s Aide, or Certified Home Health Aide (CHHA), or Certified Geriatric Nurse’s Aide (CGNA). Can be a non-aide if the administrative team agrees on the candidate’s qualifications.
- Board Certified Hospice and Palliative Nursing Assistant (CHPNA).(Preferred)
- At least two years’ experience in Hospice care or Homecare with strong communication and organizational skills.
- Must have reliable automobile, valid driver’s license, and the minimum state required liability auto Insurance.
- A compassionate attitude toward the care of the sick; the ability to read, write and carry out directions, maturity and the ability to deal effectively with the demands of the job
- Minimum of one year data entry, word processing and/or medical records maintenance experience in a medical or healthcare customer service environment.
- Required Certification/Licensure:
- Preferred graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
- Knowledge of Home Care Home Base EMR system

- Attention to detail, along with excellent organizational, verbal and written communication skills.
- Exceptional customer service and problem-solving skills with both internal and external customers.
- Ability to multi-task, worker under pressure with changing priorities and short deadlines.
- Excellent computer skills.
